I have the legal and exclusive right to provide you with the attached video. In so doing I hereby grant you a non-exclusive, royalty-free, worldwide, perpetual license to reproduce, distribute, transmit, create derivative works of or publicly display the video I am submitting.

Rules and Regulations:
By submitting video content (“video”), you hereby acknowledge to abide by the following terms and conditions:
1. You hereby acknowledge that the video submitted is yours; that is, you should only submit Video content to which you own the copyright or have the express, written consent to the copyright holder. By making a submission to The Snoring Institute, through WACH FOX news website, you are representing and warranting to The Snoring Institute, that you have the full right and authorization to submit the video and The Snoring Institute is free to implement or use the video submission if it so desires, as provided or as modified, without obtaining any further permission or license from you or any third party.
2. The Snoring Institute shall not be required to treat any video as confidential, and may use any video in its business (including without limitation, for products or advertising) without incurring liability for royalties or any other consideration of any kind, and will not incur liability as a result of any similarities that may appear in future Snoring Institute operations. You waive all rights of attribution and integrity with respect to any video submitted herein. You MUST have permission to submit these videos from the person you are filming.
3. $100 Additional fee to patient for consultation.
4. Video must be submitted by May 1, 2010.
5. Questionnaire’s to be completed and submitted along with video.
6. Restrictions may apply.
7. Winner must be a qualifying candidate. Patients with a confirmed or likely diagnosis of sleep apnea may not be candidates.
